Tom Hawkes is a transformative leader in the value-based care industry with over 20 years of healthcare leadership experience and over 7 years in ventures. He has played pivotal roles in two successful exits: Caravan Health to Signify Health and Signify Health to CVS Health, demonstrating his expertise in scaling organizations and driving innovation.
Tom’s career includes leadership roles at The Queen’s Health Systems and Kaiser Permanente, where he spearheaded strategies to improve patient outcomes and operational efficiency across diverse healthcare settings. His passion for leveraging data-driven technology to facilitate the transition to value-based care has positioned him as a trusted expert in empowering organizations to deliver better patient and provider outcomes.

Karin Abraham is a dedicated leader in value-based care, population health, and healthcare technology, with over 20 years of experience driving transformative change in the healthcare industry. She has a proven track record of developing and implementing innovative solutions that improve health outcomes and operational efficiency.
Karin’s expertise extends to her role as Senior VP of Information Systems Delivery at Caravan Health and Signify Health, where she led EHR integrations for over 200 clinical interfaces and oversaw quality reporting for more than 200 clients and 10 ACOs. Her career spans leadership in strategic planning, technical delivery, and data integration, always with a focus on enabling success in value-based care models.

ValueCycle is a joint venture between Abundant Venture Partners, Oak Street Health, First Trust Capital Partners, and 121G.
Oak Street Health’s Revenue Cycle Management team realized there was a gap between the HCC codes captured in clinical documentation and what was represented on return files from payers. OSH began the process of identifying, reconciling, and recovering value-based payments across multiple systems and payor contracts, which required significant time and resources when done manually. Over the course of 5+ years, Oak Street developed an Excel-based solution that delivered strong results for their internal revenue cycle operations, and the team believed greater efficiency could be achieved if incorporated into a software solution. OSH, Abundant Venture Partners, First Trust Capital Partners, and 121G collaborated on the product development for ValueCycle.
